Quality Assurance Nurse
Reports to: DON/Administrator
Job Classification: Hourly, non-exempt
Supervisory Duties: Ensures that all the needs are met for the clients as well as leads, supervises, directs staff that is on the shift at the MN Greenleaf facility.
Minimum Qualifications: Must be a graduate with an accredited LPN or RN license. Must be registered and licensed with the State of Minnesota as a LPN or RN, and must maintain a current and unrestricted licensure. He/she must be computer literate with a working knowledge of basic programs such as (Word and Excel).

General Requirements: MN Greenleaf facility is seeking an LPN to provide assistance with the activities of daily living to the residents. The LPN must be compassionate, respectful, patient, and possess a sincere desire to enhance the quality of life for the people whom he/she serve and their families.
1. Employee must be capable of exercising good judgment and ensure that MN Greenleaf will provide a safe, comfortable and secure living environment for the residents.
2. Employee must be able to read and write English at an average adult level. Employee must be respectful and considerate and able to communicate in English clearly in person and over the phone.
3. Employee must be able to lift, push, and pull 50 lbs of weight without risk of injury.
4. Because MN Greenleaf performs service 24 hours a day, 365 days a year, staff will be required to work the shifts/ schedules for which they are hired. Employee must be willing to work part –time or full time, and during half of the holidays recognized by MN Greenleaf.
5. Employee must be willing to undergo on the job training before being certified to work with residents including background check.
6. Employee must pass background check prior to providing cares.
7. Employee must be flexible and willing to adapt to changing situations in a professional manner. We require staff to have a happy nature and a positive attitude.

9. Employee must arrive for work on time and is ready to work at the time of the start of the shift. Follows meal/break guidelines.

Specific Requirements: The Quality Assurance Nurse agrees to fulfill the following nursing requirements for MN Greenleaf, but not limited to;
· Resident evaluations as delegated by the DON/Administrator.
· Assures clients continue to live safely in their unit, evaluated and recommend alternative placement to the DON/Administrator as necessary.
· Communicates the services to be provided for each client to staff responsible for delivering the services, and assures follow through by auditing, education, and validation.
· Inform personnel of changes in client’s needs and conditions.
· Maintains communication with clients, their families and staff.
· Facilitates coordination of client services with other service providers as needed.
· Is available for on-call medical questions and/or emergencies via phone.
· Update and post medication changes and physician orders as required.
· Conduct a consistent review of medication administration records and do remedial training/counseling for Medication Aide errors when appropriate.
· Coordinates staff education.
· Provides health and medication training to employees on an ongoing basis.
· Conduct in-service training as requested by the DON/Administrator.
· Assures that staff clearly understands and follow all policies and procedures in compliance with Minnesota regulation.
· Meets with staff as needed.
· Monitors staff for compliance of monthly client treatments as scheduled.
· Supervises all medication practices and medical equipment being used by residents.
· Assures that medication administration procedures are followed properly and discusses any needed corrections, errors, etc., with staff.
· Assures current and accurate list of medications are in place.
o Reviews each client’s medication sheet at least monthly and assures accuracy.
· Monitors and follows up on ordering medications and all medication renewals as well as any necessary follow-up with physicians regarding medications and/or medical conditions.
· Destroy/dispose of medications and document in accordance with regulation requirements and policy.
· Assures all physicians orders are signed and implemented with-in 24 hours.
· Stay abreast of changes to Minnesota state regulations pertaining to basic care and assisted living centers. Immediately notify the DON/Administrator and Administrator via email/ mail of any change that may affect MN Greenleaf’s procedures.
· Assures compliance with regulations and is available for federal and state licensure surveys.
· Provide other services as may be required by the MN Greenleaf DON/Administrator or Administrator.
· Communicates in staff communication book any information that staff needs to be aware of.
· Maintain client files and appropriate documents as required.
· Administer immunizations to staff and residents as required.
· Assists with compliance in meeting regulations and is available for state licensure surveys.
Job Type: Full-time
Pay: $53,000.00 - $69,000.00 per year
