Outreach Clinician Job at Connecticut Renaissance

Summary:
- The Outreach Clinician is a position that is comprised of 50% clinical work and 50% community outreach. The Outreach Clinician is responsible for working with the Greater Bridgeport community to identify appropriate clients and providing direct and indirect clinical services to individuals who have been directly impacted by the COVID-19 pandemic, as outlined in the American Rescue Plan Act grant. This position will collaborate with a multi-disciplinary team to create an individualized recovery program for each client as well as performing community outreach to identify appropriate clientele. This position will provide effective client supervision and counseling services for both individuals and groups as well as facilitate treatment plans. This Outreach Clinician will be responsible for file management as well as timely reporting.
Essential Duties and Responsibilities:
- Maintain a 50/50 ratio of providing outreach and clinical services to individuals in the greater Bridgeport community that have been impacted by the COVID-19 pandemic.
- Perform community outreach consisting of connecting with Bridgeport and St. Vincent's Hospitals, schools, primary care providers, the religious community and other nonprofits and community resources in Bridgeport to promote awareness of CT Renaissance behavioral health treatment services, welcome Bridgeport residents into this specialized care and locate services in the community where clients can be referred for 12-step support groups, basic needs, faith-based services, and general community supports.
- Meet program goals including but not limited to improving mental and behavioral health of clients and increasing client functioning to include the ability to return to school or to gainful employment and maintain or improve living situation (housing) from intake to discharge.
- Increase the number of clients connected to public or private insurance by actively engaging and assisting eligible clients in gaining public insurance, the DSS application process and connection to entitlements such as Medicaid / Medicare. Begin working with eligible clients at initial appointment and reengage client if s/he loses benefits. Appropriately meet clients' needs while maximizing third party reimbursement.
- Maintain a client caseload and flexible schedule, providing clinical assessments, individual and group therapy.
- Implement treatment that meets the need of clients specific to the impact from COVID-19..
- Secure initial insurance authorization and all required concurrent authorizations throughout care.
- Provide appropriate information to billing personnel as required.
- Complete all clinical documentation, including but not limited to integrated assessments, progress notes, group notes, contact notes, treatment plans and reviews, discharges and DDaP documentation according to policy.
- Input all clinical documentation in Connecticut Renaissance’s EHR accurately and timely.
- Maintain documentation and provide all data and information required for program reporting to the City of Bridgeport or other ARPA oversight authorities.
- Administer customer satisfaction surveys to all clients on a quarterly basis and provide completed surveys to the Clinical Performance and Outcomes Department for inclusion in agency Outcomes.
- Communicate with referral sources on the status of each client’s progress and performance within the program, including the preparation of reports.
- Participate in all required internal and external meetings.
- Provide urinalysis/breathalyzer testing when indicated and within guidelines of monitoring.
- Complete all required trainings to enhance knowledge and skill level.
- Adhere to state and federal regulations and standards of accreditation, licensure and funding sources.
- Meet and/or exceed agency outcomes and performance measures.
- Maintain working relationships with community referrals.
- Maintain documentation of travel and referral resources.
- Maintain professional licensure, certification, and continuing education.
- May perform other duties assigned by supervisor.
Required Knowledge, Skills, Abilities and Other Characteristics:
Knowledge Of:
- Diagnosis and assessment of mental health, substance use and co-occurring disorders.
- Bridgeport Community
- Fee for service and billing requirements for outpatient services.
- HIPAA regulations and other regulatory laws
- Electronic Health Record
- General office software, particularly Microsoft Office Suite or Office 365
Ability To:
- Communicate effectively both verbally and in writing.
- Manage priorities effectively to meet deadlines and demonstrate multi-tasking skills.
- Ability to employ effective organizational skills to meet goals
- Able to work independently and as part of a team
- Utilize therapeutic modalities in both group and individual settings.
- Manage client crises.
Education and Experience:
- Master’s degree with Licensure in Behavioral Health (LPC, LCSW, LMFT) in CT. Additional LADC a plus.
- Associate Licensure in Behavioral Health (LPC-A, LMSW, LMFT-A) eligible.
- Minimum of 3 years of working experience in behavioral health field.
- Experience in trauma informed care preferred.
- Experience within an Outpatient Behavioral Health setting preferred.
- Knowledge of the biopsychosocial and community impacts of COVID-19 preferred
Other:
- Must be dependable and reliable; detailed oriented; and able to interface easily with clients, staff, and referral sources.
- Must be able to operate a motor vehicle
- Position requires climbing stairs and the ability to lift 20 lbs or more
- May need to travel to other CTR locations and locations within the community
- Must successfully complete the introductory probationary employment period.
- Must have a valid CT driver’s license.
