POSITION SUMMARY

Directly responsible for supervising the day-to-day activities of a clinical team. Assures care and services provided by the team follow state and federal regulatory requirements of accrediting bodies. Assures care is provided to each patient in a safe, cost-effective manner that meets applicable standards of practice. Serves as the clinical resource provider to the team.

MINIMUM JOB REQUIREMENTS

Education & Qualifications:

Education: Graduate of an accredited Diploma, Associate Degree or Baccalaureate School of Nursing or master’s degree in social work. Preference is given to individuals with Baccalaureate Degree or higher.

Licensure/Certifications: Must have current licensure by the Georgia State Board of Nursing or licensure as a Clinical Social Worker in Georgia. Must have current CPR certificate.

Previous Experience: 3 -5+ years experience in case management and/or managerial responsibility in a health care field. Must have at least 1-3 years experience in home health care or hospice/oncology care. Preference given to national certification in a

clinical field.

Knowledge/Training: Knowledge of Medicare, Medicaid and other funding source regulations as applied to home health care services. Demonstrate knowledge and competence in advanced practice nursing or social work skills, including collaborating with interdisciplinary team. Demonstrate knowledge of home health proficiency in quality of clinical records, productivity, customer satisfaction surveys and leadership role with staff. Must be able to function independently and effectively. Requires proven interpersonal, supervisory and management skills with ability to communicate effectively. Demonstrates self-initiative,

proven decision-making skills and time management skills. Ability to operate computer equipment, FAX and copier machines as well as maintaining communication with Company telecommunication systems.

Computer Skills: Must be familiar with general use and functions of the computer, such as, usernames and password concepts; internet; e-mail; navigation of computer desktop or laptop, including starting programs, using files, and windows, effectively use navigation buttons and tool bars; ability to self-manage online HR services and online training programs.

Transportation: Must have reliable transportation.
