This is a full-time field-based opportunity in Louisiana. Qualified candidates must reside in one of the following parishes/counties: Orleans, Jefferson, Plaquemines, St. Bernard, Ascension, East Baton Rouge, East Feliciana, Iberville, Pointe Coupee, West Baton Rouge, West Feliciana, Assumption, Lafourche, St. Charles, St. James, St. John, St. Mary, and Terrebonne.
The Case Management Coordinator (CMC) utilizes critical thinking and judgment to collaborate and inform the case management process, in order to facilitate appropriate healthcare outcomes for members by providing care coordination, support and education for members through the use of care management tools and resources.
o Identifies high risk factors and service needs that may impact member outcomes and care planning components with appropriate referral to clinical case management or crisis intervention as appropriate.
o Coordinates and implements assigned care plan activities and monitors care plan progress.
o Identifies and escalates quality of care issues through established channels.
o Utilizes negotiation skills to secure appropriate options and services necessary to meet the member’s benefits and/or healthcare needs.
o Utilizes influencing/ motivational interviewing skills to ensure maximum member engagement and promote lifestyle/ behavior changes to achieve optimum level of health.
o Provides coaching, information and support to empower the member to make ongoing independent medical and/or healthy lifestyle choices.
o Helps member actively and knowledgably participate with their provider in healthcare decision-making.
Pay Range
The typical pay range for this role is:
Minimum: 19.50
Maximum: 40.10

Required Qualifications
- 2+ years experience in behavioral health, social services or appropriate related field equivalent to program focus.
- 2+ years experience with adult and/or adolescent populations.
- Case management and discharge planning experience.
- Demonstrated proficiency with personal computer, keyboard navigation, and mouse. Familiarity and proficiency with MS Office Suite applications including MS Teams, Outlook, Word, Excel, and SharePoint.
- Must reside in the assigned region(s).
- Ability to travel 50-75% of the time, within the assigned region(s).
- Must have access to reliable transportation, valid driver’s license, and proof of vehicle insurance.
Preferred Qualifications
- Medicaid experience.
- Managed care experience.
Education
- Minimum of a Bachelor's degree or non-licensed master level clinician required, with either degree being in behavioral health or human services (i.e., psychology, social work, marriage and family therapy, counseling).
