The Clinical Assessment and Eligibility/Comprehensive Screening and Service Model Registered (CAE/CSSM) Nurse functions as part of the interdisciplinary care management team and participates in service planning for home care clients and elders in long term care facilities with potential to return to the community. Responsibilities include clinical assessments of clients to determine their medical eligibility for programs funded by the EOEA and the DMA. The nurse may go to long term care facilities in southwest Boston to determine potential for discharge for patients and also does in-home nursing assessments to determine the personal care needs of home care clients.
DUTIES of a Community Nurse:
- Performs clinical assessments and screenings for home care clients in the Ethos area for programs and services funded by DMA through EOEA.
- Visits assigned long term care facilities on a weekly basis. Meets with residents with discharge potential, read charts and flow sheets, and complete required forms.
- Conducts nursing assessments and develops care plans for clients referred for personal care and reassesses as needed.
- Participates in the case management peer review of client records and provides consultation to case managers and other staff members regarding medical and nursing issues, insurance reimbursement and community resources.
- Collaborates with case managers to promote cost effective service plans and to determine need and eligibility for special programs funded by EOEA.
- Participates in waiver programs by consulting with case managers and screening appropriate clients.
- Screens for ECOP and Choices, and assesses for nursing facility eligibility in consultation with case managers.
- Reviews the initial assessment and service plan for all new home care clients and makes recommendations related to medical needs and special programs.
- Provides technical assistance and training in the screening process to hospitals, certified home health agencies, nursing facilities, ADH and PACE programs.
- Develops and implements in-service programs on medical issues for staff.
QUALIFICATIONS:
- Must have a Bachelor’s degree in Nursing or Human Services and at least three years of clinical experience, one of which must have been in a community setting; or an Associates Degree in Nursing and five years of experience with two years of community- based nursing or long term care direct services experience.
- A valid Registered Nurse’s license from the Commonwealth of Massachusetts and a Class D Motor Vehicle license or demonstrated ability to travel to designated facilities or consumer’s residences in a timely manner.
- Knowledge of the home care system and community resources.
- Ability to communicate effectively and work well with other professionals.
- Computer knowledge and skills are required.
- Incumbent must be able to carry a 15 to 20 lb. load to consumers’ homes, must be able to travel to the community to perform visits at consumers’ homes and travel under all weather conditions.
