Hours: Monday -Friday 8:00 a.m. to 4:30 p.m. Must be willing to work flexible hours including holidays and weekends, participating in an on-call rotation.

Experience as an RN commensurate with one of the following:
- One (1) year acute care experience or
- One (1) year home healthcare experience
- Wound care certification a plus!
Responsibilities:
- Be a master scheduler and great multitasker when it comes to planning, coordinating, providing, and documenting care for a geographically assigned caseload of patients from day of admission through day of discharge or death.
- Performs admission assessment and evaluation of patient within 48 hours of referral or return to his/her residence.
- Demonstrates knowledge of medications and their correct administration based on the patient’s clinical presentation.
- Be an advocate for our patients by initiating case conferences to discuss multidisciplinary team responsibilities, patient progress, plans for continued care, new problems, etc.
