The ideal candidate would be responsible for carrying out functions mainly in Adult Medicine, Specialty, and satellite clinic sites and as needed in the Pediatrics and Women Center within the RN scope of practice.

• Works closely with medical and ancillary staff to coordinate care based on the PCMH Home model.
• Utilizes nursing assessment and diagnostic skills to evaluate patients, and screening for necessary treatment.
• Required to provide urgent care and stabilization of acutely ill patients under the supervision/direction of a physician
• The RN will have the opportunity to work on and promote team-based interdisciplinary healthcare in a primary setting.
• Responsible for prioritizing patient conditions while providing nursing care, performing other clinical or ancillary duties, and achieving organizational goals and program objectives.
• May assist in the work and technical skills and work side by side with the DON, LVNs, and Medical Assistants to provide support to the medical providers.
• May assist staff in learning EHR changes and upgrades in new equipment
• Will supervise LVNs and CMAs with documentation, chart auditing, reporting, and QA activities leading to clinic accreditation (i.e. NCQA Accreditation) and use of thepatient portal.

From time to time, may be assigned to multiple sites or work extended hours.

Job Requirements:

• Current Texas Registered Nurse License with no restrictions.
• Current BLS-CPR provider.
• Current ACLS training preferred.
• One to two years of hospital experience preferred or working in a public or community health setting.
• Knowledge of nursing care methods, procedures, and safety guidelines
• Candidate must be compassionate and possess excellent customer service skills, and great attention to detail in a fast-paced setting.
• Versatile and ready to adapt to change, excellent organizational and computer skills, and experience with EHR, Microsoft programs, and other computer applications.
• Ability to communicate effectively, both verbally and in writing in English and preferably in Spanish, with providers, staff, and the public including public speaking and teaching abilities.
• Current TX Driver’s license, liability insurance, and transportation to travel from site to site.
