Strong communication skills, attention to detail, and the ability to multitask and prioritize are also essential. Prior experience with the Title IX I/DD Waiver Program preferred but not required.

Duties/Responsibilities include:

· Completing nursing assessments monthly, quarterly, and annually as needed

· Medical Reviews in Preparation for IPP meetings (changes in medication/treatments, appointments since last juncture, any new assessments needs, recommendations for care going forward)

· Reviewing assessment information compiled by the LPN (Falls Risk, Braden Scale, Side effects of medication, Health Service Summary

· Training Direct-Care Staff annually and when plan of care changes are made

· Monthly Nursing Summaries if there were changes within the month

· Overseeing AMAP training, medications passes, and administrative oversight.

· Preparing MARS

Qualifications

· Graduate from an accredited school of nursing or a WV RN license (can use a Multi-State Nursing License as well) current and good standing

· Valid Driver’s License from State of Residence

· Must be at least 18 years of age and have at least one year nursing experience

· Prior experience in the I/DD Waiver field preferred but not required

· Minimal travel may be required to assigned locations in the Millwood, Point Pleasant, and Parkersburg, WV areas.
