Patient Service Coordinator Job at Five Star Home Health Care

Five Star Home Health Care is seeking an experienced Patient Service Coordinator to join our team and help support our growth. The Intake Coordinator will be involved in intake, billing and insurance authorizations as needed.
The ideal candidate has a strong administrative background working independently and with teams located in multiple locations.
Job Duties:
- Monitor all boards, emails and faxes, review and accept/deny per policy
- Obtain/request all SOC documentation (order, H&P, F2F progress note, validate following physician). Engages sales team if all other measures exhausted to complete referral docs.
- Review/update referral date for accuracy
- Data entry for all referrals
- Perform Warm Welcome Call
- Schedule SOC and therapy evals
- Action Hospital Holds
- Final point of compliance for all SOC docs
- Perform Medicare or insurance eligibility check
Compensation: $24 - $28 per hour depending on experience
Work Schedule: 8am - 5pm

- Home Care Home Base (HCHB) experience required.
