This position requires that an individual applicant have the equivalency of a Bachelor's degree in a social service field plus five (5) years of experience with the population served gained through either personal, professional, or volunteer experiences OR a Master's degree.

Homeless Outreach Coordinator in the Lake Charles area
JOB DESCRIPTION
- Engage street- level homeless individuals and screen for services.
- Provides case management services to include participation in planning sessions with clients and linkage to needed resources.
- Assists client with admission to behavioral health treatment.
- Establish and maintain collaborative working relationships with local housing, community stakeholders, and treatment providers.
- Transport clients as needed to access services and resources. Clients in need of acute medical care or demonstrating serious psychological instability MAY NOT be transported.
- Participation in the Continuum of Care and other stakeholder meetings.
- Maintain a daily log of brief contacts, materials handed out, referrals, phone calls, emails, and networking.
- Direct all questions to your supervisor if you are unsure of how to proceed with a client or how to document activities.
- Complete all required documentation of services.
- Submit quarterly reports and stats to supervisor.
- Participates in staff meetings and in-service training as required.
- Maintains and submits proof of active auto liability insurance for privately owned vehicles used to conduct agency business and/or transport agency clients. Employees must submit proof of insurance prior to employment and each time policy is renewed thereafter. Failure to maintain active coverage and submit proof may result in disciplinary action.
