PURPOSE:

Under the guidelines of the Medicaid Waiver and alongside families, the Family Services Coordinator provides Home-Based services that support and assist individuals with intellectual and/or developmental disabilities. Home-Based services are based on the individuals needs and may include, but not limited to, giving guidance on financial management of the waiver funds, maintaining the individual’s Medicaid eligibility as well as benefits (such as SNAP or PACE), and assisting with requesting the need for adaptive equipment or home/vehicle modifications. The Family Services Coordinator will give the support needed for individuals to continue residing in their family’s home as well as in the community. In the circumstance of Agency-Based Model, the Family Services Coordinator may assist with supervising family support staff.

ESSENTIAL DUTIES AND RESPONSIBILITIES:

  • Serve as an advocate for the individual and families to assure all needed services are provided or obtained through the community.
  • Advise and provide resource information to individuals, families, schools, and agencies regarding services for the clients we serve including but not limited to financial and medical assistance, camps, and other related information as needed.
  • Assist the family/employer of record to complete the Employer of Record Implementation Strategy upon their request.
  • Ensure the FSS has entered their time sheets appropriately in the system and approve it.
  • Train, supervise, and evaluate Family Support Staff providing services to the individual/family. Ensure all FSS remain up to date with all internal and external trainings.
  • Assist Personal Support Workers (PSW) with time sheet completion, upon family’s request.
  • Assist in training the individuals, families, and PSWs to increase the individuals’ skills acquisition, upon their request.
  • Complete the Implementation Strategy for Agency Based Personal Support Worker (AKA Family Support Staff).
  • Data Entry throughout all databases.
  • Will meet the monthly billable hours, as assigned by their supervisor, per case load.
  • Complete all required paperwork and documentation.
  • Attend outside/community/networking meetings as they relate to the program and/or to Little City Foundation in general.
  • Attend and participate in staff meetings.
  • Attend trainings and workshops.
  • Meet regularly with the supervisor.
  • Other tasks as indicated by individual/family needs.
  • Maintains a current American Heart Association certification in First Aid and CPR.
  • Successfully completes Little City Foundation required training, re-training, and any additional training as required by Little City Foundation policies and practices, or as assigned by his or her immediate supervisor.
  • Other tasks as assigned by supervisor.

Bachelor’s degree in Human Services field with 1-year experience in working with persons with developmental or intellectual disabilities required. Candidate must be QIDP qualified. Must have reliable transportation and be willing to routinely travel to off site meetings or training with the individuals, their families or representatives. Must have strong oral and written communication skills.

PHYSICAL REQUIREMENTS & WORK ENVIRONMENT:

While performing the duties of this job, the employee is subject to sitting, walking, and standing for prolonged periods; frequently grasps, lifts, holds, or feels objects; occasionally stoops, kneels, crouches, or crawls. The employee frequently is required to use manual and finger dexterity and eye-hand coordination when working with persons served and handling office, medical, or household equipment. The employee is subject to assist and lift up to 30 pounds with varying amounts of assistance on a reoccurring basis. The employee is required to have corrected vision and hearing within normal range and the ability to operate a motor vehicle. Must have the ability to perform CPR and CPI. Generally, no occupational exposure to blood, body fluids, communicable diseases or other potentially infectious substances, but may require performing unplanned Category III tasks (Category II).
