We're looking for a dedicated nurse practitioner who is passionate about providing quality, compassionate healthcare to our patients. The right candidate will have a strong sense of commitment to our organization's mission, an in-depth understanding of healthcare regulations, and an appreciation for the diverse needs of our clients. We are looking for a motivated professional who will utilize their exceptional interpersonal skills to build positive relationships with patients and their families.

Responsibilities:

• Diagnose, treat, and manage acute and chronic medical conditions of children and adults.
• Manage patients in the medical home.
• Perform annual health assessments for patients.
• Work with patients on preventative care measures.
• Manage acute illness episodes of children and adults in collaboration with interprofessional teams.
• Work with patients to address the social determinants of health.
• Treat conditions; prescribe medications and nutritional supplements; order, perform, review, and interpret diagnostic testing such as laboratory studies, x-ray studies, electrocardiograms (EKGs), immunizations, Pap smears and breast exams; administer injections and immunizations; assist in surgical and post-surgical procedures.
• Perform office procedures such as dressing changes, physical therapy treatments, wound care, suture removal/closure as necessary.
• Manage preventive care including pediatric health screenings.
• Conduct on-call schedule in collaboration with other members of the medical staff.
• Provide instructions to patients who are prescribed self-care such as wound care and.

Job Type: Full-time

Pay: $109,408.00 - $115,882.00 per year
